Licensed, experienced social workers and case managers are available to help you and your family cope with illness, discuss your concerns and connect you to hospital and community resources.
Case managers are specially trained nurses who serve as patient liaisons providing information and support for patients and their families. They help coordinate and review the care plan for patients.
Social workers assist patient and their families in developing a comprehensive and effective discharge plan by assessing the individual medical and psychosocial needs of each patient. Discharge plans may include: home healthcare services, outpatient rehabilitation services (physical, occupational and speech therapies), short-term rehabilitation, sub-acute care and skilled nursing facility placement.
Planning for your continued recovery should begin early in your hospital stay. Discharge plans are also coordinated with your health insurance, as some services may not be covered.
Social workers and nurse case managers are available 7 days a week to answer questions, consult with family members, offer counseling and make community referrals as needed.
